- I agree to pay the fees stated in the summary below.
- All tuition fees are annual amounts that can be paid in one lump sum due August 1st or in ten equal
installments due on the 1st of each month. All closures during the academic school year have been calculated
into the tuition; therefore, no adjustment is made for those closures.
- I agree that monthly installments are due by the 1st of each month and payments are late after the 5th of each

Tuition Express platform or by enrolling in autopayment.
- I agree to pay full tuition even if my child is absent. There will be no credits for sick days, absences, or
- If payment has not been received and is late 10 days, the student(s) shall be subject to dismissal. Any exception
must be made in writing. Terms and conditions of payment must be fully and promptly made.
- New Generation Montessori reserves the right to send delinquent accounts to a collection agency. If your
account is not paid in full and it is turned over to a collection agency and/or attorney, I agree to be responsible
for all reasonable fees necessary for the collection of the delinquent account including, but not limited to, due
costs and reasonable attorney’s fee of 33% of the balance.
- The terms of this agreement are subject to a yearly increase to cover rising living expenses.
- The terms of this agreement are subject to change by New Generation Montessori without previous notice.
- Complete payment must be made by the first of the month. Parents/guardians sharing responsibility for
payments must work out solutions between themselves. The school will not intervene in conflicts between
custodial parents concerning payment or communication.
- Verbal agreements are not accepted. Any change to this tuition agreement must be made in writing and signed
by the school and the parents.
- Refund Policy: I agree that no refunds will be made to our account and any cancellation of services should be
requested in writing at least 30 days prior to the termination of services. In the event of cancellation of
services, no partial or prorated refund will be issued. I also agree that the registration fee is a non-refundable

How Do Recurring Payments Work?
You authorize regularly scheduled charges to your credit card. You will be charged the
amount indicated below each billing period. A receipt for each payment will be emailed to
you and the charge will appear on your bank statement as “New Generation Montessori”. You
agree that no prior notification will be provided unless the date or amount changes, in which
case you will receive notice from us at least 10 days prior to the payment being collected.

I (we) hereby authorize New Generation Montessori to initiate credit card charges to the above-referenced credit card account indicated above for on the 1st. of each
Month for the payment Monthly tuition plus any additional charges as stated on the Fees and
Financial Agreement. To properly affect the cancellation of this agreement, I (we) are required to
give 10 days written notice.

I Give permission to all the staff of New Generation Montessori to call “911” for emergency treatment of to my
child. In the event of an emergency in which I cannot be reached, I give consent to
transport by ambulance if the situation warrants it. I also give permission to the emergency personnel to
administer first aid and to transfer my child to a hospital if required.
If emergency medical attention (CPR and/or First Aid) is needed for my child, I authorize
New Generation Montessori to call an ambulance to transport my child for medical treatment to the closest hospital or
medical facility. I authorize New Generation Montessori staff who is trained in pediatric first aid and CPR to administer
pediatric first aid and CPR if needed. My child’s enrollment information and health information may be viewed by staff,
on a need-to-know basis, and by state licensors for compliance

appear on their printed newsletter, website, and social media channels as part of a media publication on New

Alternate Nutrition Plan
In accordance with the Palm Beach County Ordinances, parents/guardians and Child Care Providers are urged to work cooperatively
to assure that children are provided with nutritious snacks and meals when they are not offered by the Provider. Information can be

Attendance and Sick Child Policy
Please remember it is REQUIRED to keep your child at home if the following conditions exist:
A fever of 100.4 or above
A skin rash that has not been identified by a phone call or in writing by a doctor
Diarrhea and or vomiting two or more times in a day
Evidence of head lice (without proof of treatment)
Excessive runny nose
Conjunctivitis (pink eye)
Sore throat or difficulty swallowing
Infected skin patches
Pain of which the child complains and interferes with normal activity
Evidence of infection
A moist or open sore
Children ill because of communicable diseases must have a written statement from a doctor stating that they are no longer
contagious before returning to school. All children sent home must be symptom-free for 24 hours per the childcare ordinance
before returning to school. We will not accept ANY STUDENTS at the front if the students have any symptoms or if the student left
the previous day with symptoms.
Medical appointment of a child or another family member

Participation in Food-Related Activities Policy
To comply with Child Care Licensing and Enforcement Ordinance 65C-22.005(1)(c)2, parents and legal guardians of children
attending New Generation Montessori must be advised in advance that children may participate in food-related activities
during the school year. Parent consent to participate in special occasion food events such as group snack provided by a parent
(Smart School Snacks), birthday celebrations and/or similar events where food will be served, must be obtained in writing.
Consent must also be obtained for children to participate in learning activities where food may be consumed, such as
classroom cooking activities. Signed consent forms will be maintained in each child’s file (one consent form must be
completed for each child enrolled in the program)

Classes are subject to availability. All classes last between 30-45 min each.
I understand that I can withdraw my child at any time. If a session is canceled, an additional session will take place at the end
of the series. There will be no refund or credit for classes missed. First payment is due in full, at time of registration. Activity
fee will be added to your account by the 1st of each month with the regular tuition payment.
I have read and understand the information provided on this form and waive any right to claim against New Generation
Montessori and /or the instructors.
I understand the inherent risks in participating in the programs marked above. I will not hold New Generation Montessori or
any representatives responsible (or liable) for any accidents during normal class participation. I understand that New
Generation Montessori staff will do everything possible to protect my child from accident or injury.
Total Monthly Amount is $60.00 monthly per each activity selected.
School Supply list
Here is our school supply list for the 2022-2023 school year. Each item on the list is very important to
the functioning of our classroom throughout the year. If you are unable to get something on the supply
list or have any questions, just let the front office know. If you are willing and able to do so, we always
appreciate extras. The more the merrier!
Please DO NOT label the individual items with your child's name, our classrooms are a community,
and we share our supplies. Only label the bag with all the items inside it.
